Disability Income Benefits Amplify Publishing Group offers comprehensive short- and long-term disability income benefits to our employees. We are committed to protecting you and your family, which is why we cover the full cost of both short- and long-term disability insurance. This means you have no out-of-pocket expenses. This year, Guardian continues to be our provider for disability income benefits. If you become disabled due to a non work-related injury or sickness, these benefits will partially replace your lost income. Below is a brief overview of the key features of the Guardian Short-Term and Long-term Disability insurance plan.

Injury (accident): On the 1 st consecutive day of disability. Sickness (illness): On the 8 th consecutive day of disability. Or the day following the number of accumulated sick days applicable to the employee The weekly benefit is an amount equal to 60% of covered earnings, up to a maximum benefit of $1,000 per week. Maternity is covered as any other illness. Non-Occupation coverage Partial Disability benefits included. Transfer of Coverage provision

Day Benefits Begin

90 consecutive days of total disability

The monthly benefit is an amount equal to 60% of covered earnings, up to a maximum benefit of $6,000 per month. Pre-existing Conditions: 3/12 Exclusion Mental Nervous: 2 years Substance Abuse: 2 years Integration: Full Family Minimum Benefits Payable – Greater than 10% or $100
