To ensure you are maximizing these benefits, we have prepared this Open Enrollment Guide. Open enrollment is a brief period each year when you can modify your benefits. This guide details all the benefits offered by Mil-Tek USA Recycling & Waste Solutions LLC, allowing you to determine which options are best suited for you and your family. Mil-Tek US supports the medical, dental, and vision coverages by contributing 50% of the employee cost, and 50% of the additional cost for dependents. Benefits are effective on the date of employment. Changes made during open enrollment will become effective on November 1 st , 2025. Who Is Eligible? Suppose you’re a full-time employee or a part-time employee with benefits according to your contract at Mil-Tek USA. In that case, you’re eligible to enroll in the benefits outlined in this guide. How to Enroll Are you ready to enroll? The first step is to review your current benefits. Have you recently moved or gotten married? Verify all your personal information and make any necessary updates in Employee Navigator. Once your information it is time to make your benefit choices. The choices you make during open enrollment can significantly Impact your life and finances, so it is important to carefully consider your options. When to Enroll Open enrollment begins on October 20 th and runs through October 28th. The benefits you choose during open enrollment will become effective on November 1 st , 2025. How to Make Changes Unless you experience a life-changing qualifying event, you cannot make changes to your benefits until the next open enrollment period. Qualifying events include: Marriage, divorce, or legal separation Birth or adoption of a child Change in child’s dependent status. Death of a spouse, child, or other qualified dependent Change in residence.
Change in employment status or a change in coverage under another employer-sponsored plan
What’s new for 2025? Health Insurance – Medical
We will continue with United Healthcare as our medical provider. However, for the forthcoming contract period, Miltek-USA will be offering only one medical plan . We have determined that this is the most favorable option to keep the medical benefit coverage affordable for both the company and for you, our valued employee. Below you will find a chart outlining the main benefit structure of this plan. For more detailed information regarding the benefit structure, please refer to the carrier plan summaries

Health Insurance - Dental In addition to protecting your smile, dental insurance helps cover the costs of dental care, including regular checkups, cleanings, and X rays. Several studies indicate that oral diseases, such as periodontitis (gum disease), can impact other parts of your body, including your heart. By receiving regular dental care, you and your family can avoid the high cost of dental disease and surgeries.

For the services highlighted above, Delta Dental will pay the plan allowance minus any copay. Your payment responsibility is listed on the Schedule of Benefits, in addition to any amount over the benefit maximum.

Step 1: Log In Go to www.employeenavigatorcom and click Login
Returning users: Log in with the username and password you selected. Click Reset a forgotten password.
First time users: Click on your Registration Link in the email sent to you by your admin or Register as a new user. Create an account, and create your own username and password.
Step 2: Welcome! After you login click Let's Begin to complete your required tasks.

Step 3: Onboarding (For first time users, if applicable) Complete any assigned onboarding tasks before enrolling in your benefits. Once you've completed your tasks click Start Enrollment to begin your enrollments.

Step 5: Benefit Elections To enroll dependents in a benefit, click the checkbox next to the dependent's name under Who am I enrolling?

Step 6: Forms If you have elected benefits that require a beneficiary designation, Primary Care Physician, or completion of an Evidence of Insurability form, you will be prompted to add in those details.

Step 7: Review & Confirm Elections Review the benefits you selected on the enrollment summary page to make sure they are correct then click Sign & Agree to complete your enrollment. You can either print a summary of your elections for your records or login at any point during the year to view your summary online.
